appointments-portOffice hours are by appointment only, Monday, Tuesday, and Friday. Phone consults are also available, under the condition that you are under the care of a primary care provider in your area. In order to assure efficient and timely service, we are currently not accepting walk-in patients. If you’d like to make an appointment,

First Visit – Please arrive 15 minutes early to complete the initial intake forms. Alternately, you may download our initial intake form and health information form and complete the forms prior to your first visit. When you meet with me for the first time, you will typically spend about an hour and a half together. The first hour will be spent discussing your current and past medical history, which is then followed by a pertinent physical exam and discussion about possible diagnostics and treatment options, if necessary. We will then decide on a comprehensive treatment plan and I will make recommendations for follow-up.

Follow-Up Visits – The follow-up visits typically last 45 minutes, and involves an assessment of how effectively the treatment plan is working. We will also review any diagnostic results that may have been ordered during the previous visit. Any necessary adjustments will be made to the treatment program at this time, and we will continue to remove any obstacles to your cure that may be present. In addition, a thorough physical exam may be performed to aid in the prevention of future health problems.

Payment Policies – Payment is due in full at the time of service. Patients who are covered by Anthem Blue Cross/Blue Shield, Oxford, Cigna, and Connecticare health insurance plans may owe a co-pay or co-insurance amount for the visit. Patients covered by other health insurance plans will be provided with the appropriate documentation to self-file with their insurance company.
